The Summary of Fracture Risk Factors is a resource for family members of LTC residents that quickly highlights key risk factors for fractures.  It helps to categorize long term care residents to determine if they may be at high to very high risk for fractures. The more risk factors a resident has, it may help determine if they are high or very high risk.

The Ministry of Health and Long-Term Care provides information and material on long-term care and health services within the province of Ontario.

Osteoporosis Canada

Osteoporosis Canada offers a number of resources for osteoporosis including treatment, how to exercise and stay nutritious, facts & statistics, and what to do after you’ve fractured a bone. Upon visiting the site, please refer to the first main tab ‘Osteoporosis & You’ and its drop down menu options, and the second main tab ‘Programs & Resources’, and the first four drop down menu option.
